First time Applications:

Two photographs- The Probate Court does not take photos. Most CVS and Walgreen Pharmacies and post offices take passport photos. Check your local store for hours and fees.
Photo identification- This is often a Drivers License

Adults (Age 16 and over): $110.00
Minors (under age 16): $80.00
Execution fee – This is $25 made payable to the Treasurer, State of

Other information
Anyone applying for a passport for the first time must apply
in person at the Court or another Passport Acceptance Agency. If you need
to renew your application this may be done through mail. You can download
an application from the Department of State website (http://travel.state.gov/passport)
or pick one up at the Court or the Town Hall reception desk.
The Farmington Probate Court has taken on this role as a service to the local community. We understand that assembling the necessary documentation may sometimes be difficult.
