Please call the Court to schedule an appointment between the hours of 10-3:30 on Mondays, Wednesdays, and Fridays.

First time Applications:

What do you need to apply? 

An application form - This can be downloaded from the Department of State website (http://travel.state.gov/passport).  We also have them at the Court and at the reception desk of the Farmington Town Hall

                                                -The form is DS-11

Proof of United States citizenship – This is often a certified birth certificate.

Two photographs- The Probate Court does not take photos. Most CVS and Walgreen Pharmacies and post offices take passport photos. Check your local store for hours and fees.

Photo identification- This is often a Drivers License

Fees:

Application fee- payable by check or money order made out to the United States Department of State

            Adults (Age 16 and over): $110.00

            Minors (under age 16): $80.00

Execution feeThis is $25 made payable to the Treasurer, State of CT

There are additional fees for expediting the application.

Other information

Anyone applying for a passport for the first time must apply in person at the Court or another Passport Acceptance Agency.  If you need to renew your application this may be done through mail.  You can download an application from the Department of State website (http://travel.state.gov/passport) or pick one up at the Court or the Town Hall reception desk. 

The Farmington Probate Court has taken on this role as a service to the local community.  We understand that assembling the necessary documentation may sometimes be difficult.  Feel free to call the Court with questions and we will be happy to assist you as best we can.  We do encourage you to review the Department of State’s website at http://travel.state.gov/passport in order to review the requirements and prepare the application.
